GAO urges federal agencies to do more to stop at least $180B of improper payments made in error

 The Government Accountability Office (GAO) has released a report highlighting the ongoing problem of improper payments made by federal agencies, which is estimated to cost taxpayers about $186 billion in fiscal year 2025 alone. These issues have been a persistent burden, totaling around $3 trillion since 2003.

• Improper Payments: The report identifies that improper payments can include errors such as overpayments, underpayments, and payments without sufficient documentation. Many of these mistakes arise from issues like administrative errors, eligibility verification failures, and duplicate payments rather than outright fraud.

• Affected Agencies: Seven federal agencies have shown improper payment rates exceeding 10% consistently between 2021 and 2024. Notable agencies include the Departments of Labor, Education, Health and Human Services, Treasury, and Agriculture. These agencies often lack effective internal processes for reporting and monitoring payment inaccuracies.

• Impact of Healthcare Programs: Programs related to healthcare are primarily responsible for the improper payment totals, with Medicaid being a significant contributor due to longstanding documentation issues.

• Need for Reform: GAO emphasizes that instead of merely identifying these payment errors, agencies need to implement stronger prevention systems. Recommended measures include better tracking and monitoring, clearer reporting guidance, expanded corrective action plans, and urgent adherence to GAO's past recommendations.

• Oversight and Accountability: The report asserts that without improved guidance from the Office of Management and Budget (OMB) and timely reporting from agencies, Congress will struggle to oversee federal spending properly. The GAO calls for stronger oversight to improve accountability and address payment integrity effectively.

The GAO’s findings illustrate a substantial and ongoing problem with improper payments in federal agencies, necessitating significant reforms and improved oversight systems to safeguard taxpayer money and enhance operational integrity within government programs.
